To get started, the employer should complete our Request for Fund Quote information request. Employers under 200 employees must share current census and benefit information. Quotes for large employers over 200 employees require additional claims and enrollment data.

The Fund will work with its carriers to provide a complete benefit proposal to the assigned USW Staff Representative to use per their collective bargaining strategy. If our proposal is negotiated and accepted, the employer will enter a Participation Agreement with the Fund. We will work with them directly to implement coverage and provide open enrollment instructions.
