You can enroll within 30 days of a qualifying event or during your employer’s open enrollment period. The Fund must accept enrollment from your participating employer. Please enroll through your company’s HR or benefits department.

Your participating employer should have a hard-copy supply of Summary Plan Descriptions for distribution. However, if you need an electronic version or need additional hard copies, please contact your group’s Account Executive or submit your request through our Contact Us page.

Your employer is responsible for making employee contribution payroll deductions. Contact your HR or benefits department with questions about your premium contributions.

Contact Member Services at the number on your ID card. Contact your participating employer’s Account Executive with escalated claims issues.

Call the Fund Administrative Office at 1-888-296-7493 to confirm your mailing address and order new ID cards.
